The first step to becoming a new patient of The Center for Fully Functional Health or the Fully Functional MedSpa is reviewing our website to ensure our mission and methods are in line with your needs. To schedule your first appointment for Functional Medicine or MedSpa services, please complete the contact form at the bottom of this page or call our office Monday to Thursday, 8AM-5PM Eastern.
Please note, most MedSpa wellness and aesthetic services are available without a Physician consult. Patients in need of more complex care or Physician appointments will need to complete an onboarding call with our New Patient Support team, where we will answer any questions you may have, ensure our practice is the right fit, and schedule appointments as needed.
As a reminder: Dr. Ellen and Dr. Scott Antoine are Functional Medicine Physicians who work in conjunction with your current healthcare team to address chronic health issues. Their care is not a replacement for your Primary Care Physician. As such, we require all new Functional Medicine patients to have a Primary Care Physician to cover emergencies and routine care and screening. If you do not currently have a PCP, please ask our team about joining our Free Climb Direct Care Membership program.
Many MedSpa wellness and aesthetic services are available without a Clinician consult. Patients in need of more complex care or Physician appointments will be guided through the onboarding and appointment process by one of our New Patient Support Coordinators. We do everything we can to make the scheduling and treatment process as simple and nurturing as possible.
Functional Medicine appointments are conducted both virtually and in the office, depending on the need for physical examination or laboratory testing. MedSpa treatments are always conducted in person.
Our goal is to provide you with the highest level of personalized care possible. We are committed to partnering with you to achieve optimum health.
After your New Patient Appointments are scheduled, we will send you a link to log into the patient portal. It is VERY important to log in prior to your appointment and complete your enclosed patient forms and questionnaires. The patient portal will be the main access for communication with the office.
Having these forms filled out in advance (3-5 days prior to your scheduled appointment) is required for the most efficient use of time together and will allow our Physicians to spend more quality time with you during your appointments.
Medical records can only be released with your authorization. A release of information form will need to be filled out. It is your responsibility to obtain previous medical records from other Physicians or Healthcare Clinicians that you wish one of our Physicians to review. Please contact your healthcare Clinician and obtain these records or have them fax them to our office 877-257-4003 and make sure that we have received them at least seven business days prior to your initial appointment.
All phlebotomy and laboratory testing services are available at our office through our Labcorp satellite office, reserved exclusively for the use of our patients. Any specialty test that requires blood (and sometimes urine) will be obtained and sent off from our office. Some specialty lab tests will need to be completed at home, such as saliva and stool tests.
All of the supplements recommended at CFFH are available for purchase in our office Fully Functional Apothecary or from our patient portal. If you would like to pick up supplements from the office outside of your scheduled appointment, please stop by during our office hours or contact our office through the patient portal or at [email protected]. You can also have supplements shipped to your home.
Cash, checks, credit cards (MasterCard, Visa, Discover), or Care Credit* are all accepted methods of payment for our Functional Medicine services. We also offer Cherry as a payment option for Aesthetic services.
Your New Patient Support Coordinator or the Office Manager will review your payment calculations and payment schedule based on your customized treatment plan. In general, payment is due in full before any scheduled service or appointment, though some longer-term care plans feature installment payments.
*Care Credit works just like a credit card, but it is exclusively for healthcare costs. It allows up to six months same as cash before interest charges are applied. With low monthly payments every time you use it, you can use your card at each follow-up appointment or MedSpa treatment session. Cherry works similarly for Aesthetic treatments.
We are committed to being on time for our patients. Please come to in-office appointments at least 10 minutes prior to your scheduled time in order to complete the check-in process. If you arrive late, we may be required to reschedule your appointment, and you may be charged for the original appointment time.
Functional and Integrative Medicine is a direct pay model. Medical insurance is not accepted for Clinician appointments, and our office cannot assist you with claim resolution. Our office has opted out of Medicare, and you should not and may not submit your paperwork to them for reimbursement. If you are a Medicare patient, you will be asked to sign a form attesting to your understanding of our Medicare policies prior to or at your visit.
All other patients will be provided a complete billing receipt (Superbill) that can be submitted to their insurance company for reimbursement. If or how much they reimburse is variable and you will need to check with your insurance carrier. We cannot guarantee you will be reimbursed at all.
We are open Monday to Thursday, 8 AM-5 PM, at 40 North Rangeline Rd, Carmel, IN.
Should you reach our voicemail, please leave your full name, birthdate, phone number, and reason for your call, and we will respond at our earliest convenience. Messages are typically responded to within 1-2 business days.
We are not available for emergencies. If you have a medical emergency, please call 911 and/or go directly to the nearest Emergency Room (ER).
The patient portal is the best way to communicate with our office staff. Please utilize this benefit whenever possible. Our Physicians will only respond to patient-related emails within the patient portal.
For prescription refills, our Clinicians may require an appointment, depending on how long it has been since you were last seen. If they are willing to call in a refill, we ask that you have your pharmacy fax a medical refill request to (877) 257-4003. You may also request a refill through the patient portal or by calling our office at (317) 989-8463. Please note, it may take up to 72 business hours to process your refill request, so please plan ahead to make sure your medications do not lapse.
